AUTHORIZATION FOR USE OF PLAYER IMAGE & LIKENESS

I, hereby grant permission to Combat Baseball Academy to capture, use, and distribute photographs, videos, and other media featuring my child for promotional, marketing, and instructional purposes.

SCOPE OF MEDIA USAGE

I understand that my child’s image, name, and/or likeness may be used in the following ways:

● Social media platforms

● Printed materials such as flyers, brochures, and newsletters

● Digital promotions, advertisements, and highlight reels

● Local media coverage related to Combat Baseball Academy events

WAIVER & RELEASE

I acknowledge that Combat Baseball Academy will not provide financial compensation for the use of my child’s image or likeness. I also understand that all media content remains the property of Combat Baseball Academy and may be used indefinitely unless revoked in writing by me.

Additionally, I release Combat Baseball Academy, its coaches, staff, and affiliates from any liability related to the use of my child’s image in accordance with this media release.

PARENT/GUARDIAN CONSENT

I AGREE to allow my child’s image and likeness to be used as described above.
